Safe-MED (Safe Medication Use Through Education on Deprescribing for Older Adults)

This series of educational modules aims to educate healthcare professionals on the importance of optimal age-friendly approaches to medication deprescribing decisions to guide medication safety through targeted harm reduction and incorporating best practices.

  • Monitoring and Supporting Patients After Deprescribing

  • Key information for providers working with older adults and their care partners to optimize their medication plans
  • Recommendations for addressing deprescribing
  • A reference sheet for future use
  • Resources to learn more
