SAFE-MED– Medication Deprescribing Decisions Educational Program for Healthcare Providers
Safe-MED (Safe Medication Use Through Education on Deprescribing for Older Adults)
This series of educational modules aims to educate healthcare professionals on the importance of optimal age-friendly approaches to medication deprescribing decisions to guide medication safety through targeted harm reduction and incorporating best practices.
Modules available in this series:

- Monitoring and Supporting Patients After Deprescribing
Learning Objectives
Each 20-minute module includes:
- Key information for providers working with older adults and their care partners to optimize their medication plans
- Recommendations for addressing deprescribing
- A reference sheet for future use
- Resources to learn more
This course was written for the E4 Center by Alexis Coffee, PharmD and Abigail Elmes, PharmD from the University of Illinois at Chicago College of Pharmacy, in collaboration with Magdalena Bednarczyk, MD and Daniel Maher, NP from Rush University Medical Center Geriatric Medicine.
Free continuing education is available for providers who complete all three modules.
